HEBEIA — Terms and conditions — Concierge services Version: 2026-10-20-sur-devis 1. Seller and purpose TENET HOLDING, a French SASU with capital of €100, Paris company register 103 004 859, 6 rue d’Armaillé, 75017 Paris, bonjour@hebeia.life, provides concierge services under the HEBEIA brand: preparation, organisation, coordination and administrative follow-up of health pathways. These terms govern only those services. 2. Service scope The single HEBEIA — Longevity Programme package includes twelve months of concierge support: practical preparation, scheduling the initial appointment with Dr Wassim Badiou at Cyrnos, collecting professional fees and preparing an estimated medical budget, coordinating the physician-selected assessment, organising the results appointment, three follow-up consultations and agreed repeat tests. Grouping the assessment into one or two days is a scheduling objective, subject to medical indications and availability. Concierge support covers practical correspondence, reminders and appointment changes on working days. Consultation fees, including initial, results and three follow-up appointments, are excluded. The package includes no medical on-call or emergency service, permanent in-person escort, transport or accommodation, or coordination of a new complex pathway outside the agreed programme. Additional concierge services require an accepted separate quote. There is no subscription or automatic renewal. 3. Prices and separate services Concierge services are offered exclusively by personalised quotation. The quote specifies the scope, duration, amount excluding VAT, VAT at 20%, total including VAT, validity period and payment terms. It covers only the concierge services described in Article 2. No payment is requested before the quote is accepted. Medical fees, laboratory tests, imaging, exercise tests, care, treatment, medicines, supplements, transport and accommodation are excluded. Professionals contract with the client and set, bill and collect payment for their own services directly. HEBEIA neither purchases nor resells medical acts and collects no funds on behalf of healthcare professionals. The concierge quote is separate from medical quotes. The initial consultation is quoted before the appointment. Following that consultation, an estimated medical budget is prepared from the selected programme and professionals’ quotes. Each paid medical service requires a quote and the client’s prior approval. The estimate is not an all-inclusive medical package. Additional paid concierge services require an accepted supplementary quote. 4. Order and payment Requesting a quote carries no commitment. Before ordering, the client receives the personalised quote and these terms in a form they can keep, and reviews the scope, exclusions and payment terms. The contract is concluded when the client expressly accepts the quote and these terms in writing. Payment follows the terms specified in the quote; an individual payment link may be sent after acceptance. Payment covers concierge fees only, with no collection mandate for healthcare professionals. Confirmation and contractual documents are provided on a durable medium. The TENET HOLDING invoice details VAT; any Stripe receipt does not replace it. The terms can be downloaded before ordering. HEBEIA requests no medical data in the payment form. Clients must provide accurate billing details. No automatic renewal is triggered. 5. Performance and coordination The twelve-month service begins on the date agreed in writing with the client. By default, no service starts before the fourteen-day withdrawal period has ended. An early start requires the client’s express written request. HEBEIA performs the agreed coordination work and remains responsible for its own services. Professional availability may require scheduling changes agreed with the client. Dr Wassim Badiou provides medical leadership for the proposed pathway. Clients remain free to choose professionals. The physician alone decides which tests and care are appropriate. No biomarker count, diagnosis, prescription, medical outcome or unconfirmed appointment date is guaranteed. 6. Withdrawal For a distance order, the consumer has fourteen days from contract conclusion to withdraw without giving a reason, by an unequivocal statement to bonjour@hebeia.life or TENET HOLDING’s postal address. The form below may be used but is not mandatory. If the client expressly requests performance before the fourteen-day period expires and then withdraws, only a proportionate amount for the service actually performed is payable under Article L.221-25 of the French Consumer Code. For a fully performed service, the right is lost only with prior express consent and acknowledgement of that loss after full performance. Any refund due is made within fourteen days of notification of withdrawal using the original payment method, unless another method without fees is expressly agreed. 7. Model withdrawal form To TENET HOLDING, 6 rue d’Armaillé, 75017 Paris, bonjour@hebeia.life: I hereby withdraw from the contract for the following concierge service: […]. Ordered on: […]. Quote or order reference: […]. Consumer’s name and address: […]. Date: […]. Signature required only for paper submissions. 8. Rescheduling and cancellation Requests to reschedule or cancel must be sent to HEBEIA. Outside statutory withdrawal, only concierge services actually performed remain payable; their value is explained to the client and the unperformed balance is refunded. Agreed rescheduling does not automatically extend the contractual duration. Professionals’ own conditions apply separately to their appointments and services. HEBEIA holds no medical fees and cannot refund services paid to a third party. If HEBEIA cannot provide an agreed concierge service, an alternative is proposed with the client’s agreement or the unperformed part is refunded, without prejudice to statutory rights. 9. Data and confidentiality Administrative data needed for quotes, payment and coordination are processed under the privacy policy. Results, prescriptions and medical reports are exchanged directly with healthcare professionals. Clients must not send medical documents to HEBEIA. 10. Complaints Complaints should be sent to bonjour@hebeia.life or TENET HOLDING, 6 rue d’Armaillé, 75017 Paris. These terms do not limit the consumer’s statutory rights or remedies. 11. Applicable law French law applies, subject to mandatory consumer protections and applicable jurisdiction rules.
